Insurance Information

 

We generally accept the insurance plans listed below, however not all plans will be in-network. We are required to check in-network status, but we cannot guarantee the accuracy of the information we receive from the insurance companies, so please verify directly with your insurance provider (by creating an online account with them or calling the number on the back of your card) that Brent Jaster MD, JasterHealth Preventive & Wellness Medicine or L.I.F.E. Medicine Institute is listed as in-network for your plan. If you are seeing our other providers, they will be in-network if any of the above are in-network.

We have a billing service that deals with insurance claims, but often insurance companies will process claims differently than the benefits they quote to us initially, and we do not have the administrative resources to research each plan in detail. We accept insurance as a courtesy, so we ask that patients be responsible for understanding their insurance plan and researching what they will owe as out-of-pocket expenses (copays, coinsurance, laboratory fees, prescription coverage, etc) BEFORE their visits. We are always happy to discuss billing questions if you are unsure after doing your own research.

  • State Medicaid (NOT Denver Health, Rocky Mountain Health Plans, etc) plans are accepted; No show/late cancellation is subject to dismissal from the clinic. Care in the clinic requires making your appointments. We are not currently accepting new Medicaid patients, but hope to do so in the near future.

  • As of July 1, 2020, we can no longer bill Medicare.

  • We do NOT currently accept Rocky Mountain Health Plans insurance

  • We do NOT do Worker’s Comp

Fee Schedule

 

Listed below are the fees for the most commonly billed codes. Charges submitted to insurance are not the same as what you might owe. Clinics submit charges to insurance for services rendered, and must charge the same for all insurers, and cash-pay patients. The charge and contracted rate of reimbursement may be dramatically different. Cash-pay patients are offered a prompt pay discount of approximately 20% when payment is received at the time of service.

  • New Patient Office Visit: Cost depends on the time and complexity of the visit. Typically the range is $250-$300 (usually insurance code 99205) for cash-pay patients or patients who will owe out-of-pocket because their insurance doesn't  pay until they reach the deductible.

  • Established Patient Office Visit: Cost depends on the time and complexity of the visit. Typically the range is $160-$200 (usually insurance code 99215) for cash-pay patients or patients who will owe out-of-pocket because their insurance doesn't pay until they reach the deductible.

  • Phone Visits: We understand that everyone has a busy schedule, and it can be difficult for patients to come into the office for an appointment. However, insurance rarely reimburses doctors for medical care provided over the phone. We are a small clinic with limited staff and cannot discuss medical concerns outside of appointments generally. We usually schedule 40+ minute appointments to address concerns adequately, unless it is a more urgent matter. Phone visits are not covered by insurance (with rare exception) and will be charged based on time (as outlined in our Office Policies). Please call the office if you have concerns and we will determine the best option for you.
